Responsible Employees are mandated by law to act on a reportable crime or incident that is made known to them in their professional capacity, even if the information is shared in confidence.
It is important to know that the University takes any reports or complaints of crimes/incidents seriously. The University is required to follow up on all reported incidents of sexual harassment including sexual misconduct, sexual assault, domestic violence, dating violence, and stalking to support the University’s efforts to provide a safe and non-discriminatory educational and living environment. Any reported crimes or sexual harassment will be reviewed and may be investigated according to the University complaint procedures. Please note that University procedures are separate from any criminal processes related to an incident, and Responsible Employees (including Student Health Services staff) are required by law to report certain incidents to University officials. Reports can be submitted online through the Public Incident Reporting Form.
For assistance filing a University report, please contact the Office of Equal Opportunity and Access at (309) 438-5411.

You may also download and print a Formal Complaint document to submit the required information to OEOA.
Please note, if you fill out the incorrect form, OEOA will still review the information provided and help you find the correct process.
